Service Description: This survey supports playgroundsafety inspections with the following capabilities: General Site Information: Park selection (from CSV lookup), district/region auto-population, location name, and geopoint mapping Inspection Details: Inspector info, date/time, weather conditions, temperature, qualifications, and inspection purpose (Annual or Prior to Use) Owner/Operator Records: Documentation verification for procurement, upgrades, structural testing, site plans, compliance letters, and inspection history Site Furnishings (Repeatable): Inventory of exempt equipment (benches, lighting, fencing, etc.) with condition ratings, photos, and repair cost estimates Site Layout: Perimeter, area measurements, and annotated layout photos Surfacing Information: Multiple surface types, dimensions, manufacturer details, compaction/contamination assessment, and retaining wall evaluation Fall Height & Depth Testing: Maximum CSA fall heights, equipment heights, surfacing depth measurements (minimum 3 locations with averaging) HIC/Gmax Testing: Head Injury Criteria and gravitational maximum testing at 3 locations with 3 drops each General Equipment Signage: Owner/operator contact info, manufacturer details, and age group indicators Detailed Equipment Inspections: Comprehensive checks for stairs, climbers, slides, swings, see-saws, merry-go-rounds, spring rockers, and various play structuresFeature Layers Created Main Survey Layer - Primary inspection records with geopoint Site Furnishings (Repeat) - Exempt equipment inventory Surfacing Dimensions (Repeat) - Surface measurements by type Surfacing Manufacturers (Repeat) - Supplier info by surface type Max Fall Height (Repeat) - Equipment fall height records Depth Surfacing (Repeat) - Protective surfacing depth tests HIC/Gmax Locations (Repeat) - Test location records HIC/Gmax Drops (Nested Repeat) - Individual drop test results
